Lumbar transforaminal epidural steroid injections are a common procedure used to alleviate back pain by delivering a long-lasting steroid directly into a specific area of the spine. This targeted injection takes place through an opening on the side of the spine called a foramen, where a crucial nerve root naturally exits. By targeting the nerve root and the space surrounding it, this procedure aims to reduce inflammation and promote faster healing of herniated discs.

THE PROCEDURE: HOW LUMBAR TRANSFORAMINAL INJECTIONS ARE PERFORMED

A lumbar transforaminal epidural steroid injection is typically performed on an outpatient basis and does not require sedation. The patient is positioned face down on a standard x-ray table, and the skin above the treatment area is numbed. The injection itself should not cause any significant discomfort, as the skin is numbed using similar agents to those used in dentistry.

A needle is then inserted through the skin, and if necessary, a fluoroscope is used to guide the needle to the precise location. This guidance ensures accuracy and safety during the injection. The procedure may also be referred to as a root sleeve block, transforaminal epidural block, or root block.

RECOVERY AND WHAT TO EXPECT AFTER THE PROCEDURE

Following the injection, the patient is monitored for approximately 30 minutes to ensure there are no adverse reactions to the medications or needle placement. It is important not to drive for the remainder of the day, as the local anesthetic used can temporarily affect reflexes and response time.

The day after the procedure, normal daily activities can typically be resumed. Some soreness or tenderness around the injection site is normal but temporary and can be relieved by applying an ice pack.

WHAT TO EXPECT DURING THE TRANSFORAMINAL INJECTION

During the transforaminal injection, a thin needle is inserted through the skin and into the deeper tissues below. While some discomfort may be experienced, a local anesthetic can be used to numb both the deeper tissues and the surrounding skin before the injection needle is inserted. This numbing agent helps minimize any pain or discomfort during the procedure. Some patients may prefer to undergo the procedure with intravenous sedation, while others may opt to have the injection without any anesthesia and still tolerate it well.

Immediately after the injection, you may feel numbness or slight heaviness in the limb corresponding to the injection location. This is due to the local anesthetic used and should not hinder normal movement. Many patients experience immediate pain relief as a result of the local anesthetic, although the effects are temporary and typically last only a few hours. Soreness or achiness around the injection site may occur for a few days, but substantial pain relief can be expected around the third day.
